NHS-funded patient transport is rarely available, so we collect your loved one from the Discharge Lounge ourselves. A familiar member of our team brings them home in a comfortable vehicle, helps with bags, paperwork and medication, and supports them safely into the property with any mobility aids they need.
Once your loved one is settled at home, our experienced team carries out a full assessment covering mobility, medication, personal care needs, the home environment and family support network. From this we build a personalised care plan that reflects routines, preferences and any input from hospital discharge teams, GPs or physiotherapists.
Care visits are arranged around the new plan — typically covering personal care, medication prompts, meal preparation, mobility support and companionship. Wherever possible we provide a small, familiar care team so trust builds quickly. At the end of the first week we review what’s working and adjust the plan as needs settle.
